Assignment #1 (100 points) individual project - FEBRUARY 15.
Epidemiological Assessment: Use local, state and national data, including Healthy People 2020 goals for the epidemiological assessment. You must use County Health Rankings local data for NYC – Brooklyn, and DOH data for Brooklyn, available on the website.
Assessment Plan Grid: Develop a basic assessment plan that includes a description of the information needed, method of data collection, source (who will you get this information from), questions that will be asked, timeframe for data collection and how will you do the analysis.
Assessment tool – Develop focus group guide, interview guide and/or survey questionnaire depending on assessment methods chosen by group. All students must develop a 5 - 8 item individual or focus group questionnaire, or survey. Due on February 15, 2017.
Assignment #2 (100 points) group project – FEBRUARY 22.
Presentation of Assessment Findings or Literature Review and Assessment Tool.
Students will present the results from the literature review and assessment via a PowerPoint presentation. Due on February 22, 2017.
Assignment #3 (100 points) group project – MARCH 1.
Program goals and objectives - For each priority area develop SMART objectives – long term, intermediate, short term
Summary of Best Practices - Identify at least two interventions for each priority area that address the desired objectives. At least one intervention must be a policy or systems change. Provide an overview of the intervention and discuss why it was chosen, the target audience, activities, participant incentives, modifications to the intervention that may be necessary, stakeholders that need to be involved, timeline involved, etc. Justify why these interventions were chosen.
Due on March 1, 2017.
Revisions of the assignment #3 are due on April 5, 2017 - ONLINE.
Assignment #4 (200 points) group project - MARCH 8.
Proposed program intervention – Each committee or a team is going to write a component of a final paper for the class. Description of the individual small group or a committee work, including assessment of needs of a target group, underlying theory & logic model, planning activities, time line for implementation, evaluation, and resources/budget. There should be six (6) papers. A detailed format will be provided.
Logic Model - Develop a logic model for the entire project. Due on March 8, 2017.
Assignment #5 (100 points) group project - MARCH 22.
Evaluation plan and evaluation instrument(s), and project budget due.
Develop evaluation plan and justify why these methods were chosen
Develop a draft budget for your grant proposal using the budgeting. Due on MARCH 22, 2017.
Assignment #6 (100 points) group project – each group complementing the final presentation – MAY 3.
Summary team presentation of grant proposal including problem statement, program goals, objectives, intervention description, resources, implementation plan, and evaluation plan. One power point presentation as a summary of the revised a one 10-page (maximum) grant proposal in the assigned format due by the final exam class time in May 3, 2017.
Assignment # 7 - Service Learning Project (300 points) – APRIL 12.
Organizing the Public Health Program—Healthiest Nation 2030—at LIU Brooklyn
Each student will receive information and planning directions at the beginning of the semester. Students will participate on one of the planning committees. In addition, in order to receive a grade in the course, each student is required to contribute at least 2 hours at the registration desk and participate as a host during the event. The Public Health program will be held on April 12, 2017.
